Not at all. No one is advocating acting like an asshole.

I feel bad for US physicians. They deal with the most non-compliant, unfit patients in the world. When I round or spend a day in the office with physicians, it amazes me what comes in. There will be patients with missing segments of bone from a trauma. The doctor will carefully explain to them how if they continue to smoke, they'll almost certainly have problems getting the bone to heal. What does the patient do? They light-up the moment they step out of his office. And the say thing is, he's covered under a global fee. So the doctor gets the same $$$ for him that he gets for the fitness nut who want advice on how to help his bones heal better/faster. And when the smoker doesn't heal, there's an army of lawyers ready to take the case on a contingency basis. It's absolutely crazy.
