First, in my opinion it is possible that you have prostatitis or epidydimitis or even orchitis, that would be why you have pain in your testicles. That is my guess.
Next, don't do any HCG, that stuff just delays your system's recovery as much as anything else would.
Last, what about arimdex? Have you tried that? That is pretty potent at restoring your HPTA. About as potent as it gets I think.
Activity is also important, you should try to lose weight. If you have alot of lower body fat from previous cycles you will have a tough time ever getting your HPTA back in order. Lower body fat harbors high density concentrations of aromatase that will eat up your testosterone and adrenal androgens and also turn them into estrogens which will keep the LH and FSH levels down. Your LH levels are not appropriate in my nonmedical opinion, given your low testosterone levels.
