Great stuff, everybody. I put in another vote for professional help if the advice here doesn't help (which, based on your last post, it looks like you're committed to doing).
I'm not sure what your history is before the 4-year period you outlined, but if your fear of fat gain is based on a prior period of being overweight, you just have to realize things are completely different now. You're not like the average lazy guy who accidentally glances at a gym once or twice per year, throws down fast/junk food, soda/alcohol, etc. on a daily basis and doesn't even have the basic understanding of nutrition/physiology to understand why he's fat or what he could do about it.
Especially being anal about your diet, you're in complete control (or would be if you could overcome your mental obstacles). When you eat above maintenance and gain a bit of fat (look at it as a necessary side-effect of gaining muscle, not as "getting fat"), you'll know exactly what you need to get rid of it while still retaining most of that muscle.
