After massive weight loss, loose skin compensates only so far. It is not just an issue of residual fat, but also the nature of the skin itself. Stretched out skin can actually have broken structure such as stretch marks. How much the skin shrinks varies from person to person. When there are no stretch marks, skin may be better able to shrink as there may be less internal damage.

Getting to a weight / body fat
before considering any surgery is important. Weight loss is a coarse tool. You cannot pick where the fat comes from or goes back onto. Surgery is better reserved as a refinement tool. If planning to lose weight after surgery, what makes you think that the skin will shrink? If it is going to shrink, it is better to get to a weight you are happy with before surgery. Then surgery can refine what does not happen naturally.
Be careful about topical treatments for skin tightening, creams and lotions just do not have a good track record. In before after images, watch out for changes in lighting techniques which can hide the problem better than the supposed treatment.
Filling loose skin with bulk muscle can help and that is why the chest and upper arms / shoulder tend to be less loose in body builders than their stomach. Bigger back muscles can tighten the upper stomach to a slght degree, but tight abs just do little for loose skin.
After weight loss, the skin can take some time to adjust further. Depending on many factors, this further adjustment can take 6 to 18 months. As long as the skin is shrinking and improving, hold off any consideration for body contouring surgery. Being patient can help, but does stress the mind for some.
