Mail order's the way to go, but if ya gotta have pins fast, here's a way to try it if the people behind the counter are known assholes:
I write out a list of "grocery" type items, and include the pins I want. It lists stuff like aspirin, Gold Bond foot powder, oranges, light bulbs... you know, stuff that old people need. I make it out like it's from my aunt, all shaky handwriting. The note says, "Skepti, can you please pick up these supplies. Love, Aunt Hildred." Then, I take a different colored pen and scratch out all the items except the pins.
I call the pharmacy ahead of time and ask them if they have them in sotck. If they give me any slack over the phone, I say they're for my aunt. If they ask what she uses them for, I say she takes some B-12 injections or something, I don't really have a clue as I don't normally buy this stuff for her.
Once I know they have them in sotck, they can't say they're out of em once I get there. Then I hand them the list and explain that I just called em. They look the list over and almost never give me a second glance.
If they give me crap at the store, I start in like, "Well, geez, I don't have a clue about any of that stuff. I just know she and my uncle are taking shots all the time. Between the two of em, they're like a couple dang pincushions. He's a diabetic, and she's got arthritis or something. Needles creep me out., so I'd just as soon not see them, if it's all the same to you. Can you just bag em up?"
By then, even the most jaded old lady will sell em to me. I almost feel bad.
BTW, be sure you cross out any items you don't want to buy at the pharm. I had to buy that damn Gold Bond because I hadn't crossed it off my list, and the old lady said, "Oh, we have the Gold Bond too, here ya go."
