Never had a bad experience on myself, but had one on a patient the other day. Had to give an IM to an 88 yo F, that weighed like 90lbs. Standard where I work is a 1 1/2" 20g. it's like holding a little mini sword. Decided that the only place she had enough tissue to stick was her VL (quad). This is where it gets bad....we've gone all "safety" with our products, and our larger syringes have this safety tube sheath thing covering them so that when your done, you just slide it down over the needle. Well I'm holding the syringe, which is actually like holding just the tube thing. I pin her, she screams, I go to aspirate and see that the needle barely penetrated the skin because when it hit, it pushed the syringe out of the plastic safety sheath thing and locked it in place. Fuck! So I have to get another one and explain to her what happened. Guess what? That's right...same thing on second try. Third time was the charm, and I went straight to our supply person to rip them a new one for ordering such shit.
