Help me Rehab my shoulder!!

Hey guys, for a while now my shoulders have been hurting and sore pretty often (I think its my rotator cuff) I don't know if its something im doing that bothers it or if its cause im getting big or what?!! lol Anyways I lift doing Brazilian Jiu-jitsu and box regularly so this is obviously a problem. Most often if I warm up good they don't hurt when im practicing but after they might. I have been recently been doing like 3 sets of 2.5 pounds weight like outward pushes that works your rotator like 2 to 3 times a week? Is this all I should be doing? Should I be doing more exercises more sets? etc. Also any good stretches I need to be doing to help this and maybe Ice or what? And when should I do the stretches? Thank you for all your help hopefully I can fix this problem asap!
 
Im currently cruising on 500 mg a week. Im not sure that it is that bad that I need a MRI, because for one I still have been doing most of my regurlar activities. Do youguys think it has anything to do with the way I sleep? I sleep on my side usually, but I have allways done this.... Maybe I just got too big to sleep on my side? lol. I have been slowly trying to sleep flat it just is so uncorfmtable that I usuaally cant fall asleep.... but im tryin
 
Whats your lifting like? Any heavy bench or overhead pressing? If so stop till pain is gone. ive had shoulder surgery and it sucks! Long recovery and lots of pain, if your have any specific questions feel free to ask or pm me.

Nah it rarely hurts when I do lifting actually, I usually warmup my rotators before bench by doing like a set of something for them
