oi you, stop being unethicalBrothaBilly said:lasix, furosemide, give it to people all the time, drops fluid retention, its a loop diuretic referring to the renal loop where it dumps water, people with high bp or heartfailure dump a few pounds of fluid volume in the body system and the heart has less work to do, safe and easy if you know what you are doing deadly if you dont, old people take them like aspirin

in a young first timer, using frusemide is a silly risk. someone wiht a good, healthy kidney on even a low dose of frusemide is going to produce a LOT of urine. while im not overly concerned with vascular hydration in someone as vibrant as dakotah, youre going straight to the worst case scenario, dehydrating them vascularly rather than dermally. what for?
the otehr thing to think about is that even if she runs to the pharmacy and gets some in the next 30 seconds, whe is going to get up 8 times tonight to go to the toilet, meaning that her sleep quality is going to hell and will probably zap her performance the next day. frusemide = in the morning type drug.
i just went through this with one of my cousins, who went in a bench press competition last weekend. he popped 2 lasix without asking me first (why the hell do they do taht, when ill answer any question for them anyway? twit) and instead of lifting 160kg like he normally does, he stalled on his starting weight becuse he was zapped. a guy who reps 160 only moved 130 in front of everyone, in the under 200lb class. lol!

when you start someone out on frusemide (there are 2 ways of spelling it, in australia its predominantly spelt frusemide...trust me i was confused a couple of times and had this convo beforeBrothaBilly said:Not sure what you mean dehydrating vascularly vs. dermally, last I studied this stuff in university the dermis was fed by capillaries meaning its all part of the same system, you dont have areas that hold onto more water without an outside force. Loculated or venous or lymph issues, ie filaria worm(elaphantiasis), varicose veins etc...
ie swollen ankles or edema, gravity allows the fluid to collect, giving lasix reduces the swelling bilaterally and reduces the afterload of the heart. Youre taking fluid out of the system by changing the ionic gradient, that means fluid intra and extracellular fluid from all over. Less fluid in the capillary network the less the blood pressure of the system. I dont know, Ive popped lasix many a times, its the best blood pressure medication available according to research my man and should be used first line with all factors considered, taking something a first choice med safely is not a worst case scenario. Tons of people take diuretics, Ive taken them before to beat the old drug testing where you just piss water, but that was before I found zen.
When I drank in the past, it seemed like every so often I would retain water and get puffy faced from the alcohol, yknow, the renal alcohol effect so Id take 40mg lasix and zing, back to looking and feeling normal, thats why people that come in who drank the night before have high bps, b/c of fluid retention, just have to equalize that shit out and sitting in a sauna is going to exhaust you more than changing the ion balance of your system
Well whatever, I know how to use them but I dont need them, they are very effective and useful medication and often overlooked as a bp med b/c of pharmaceutical marketing according to that HUGE study a coupla years ago, Im sure you heard about it
and its spelled furosemide![]()
GoldenDelicious said:when you start someone out on frusemide (there are 2 ways of spelling it, in australia its predominantly spelt frusemide...trust me i was confused a couple of times and had this convo before) you obviously start producing greater urine volume, and lots of people assume that overall water lost will be lost from the different compartments of the body equally - but in fact, when the fluid loss is very rapid, much of it is lost from the blood itself, and fluid doesnt seep out of other areas quickly enough to maintain normal blood volume.
for example if you get a patient with oedema in their ankles, and slug them with a big dose of frusemide, after say, half a day, you might find that their weight has gone down 4 lbs (and you think good! the water is coming out!) but youll notice that the oedema in their ankles is only a little reduced, but their tongue and other mucous membranes have started to dry up - this indicates that the water is being sucked out of the bloodstream too quickly, and so you should slow down overall fluid loss so that the patient doesnt end up having problematically low blood pressure and exascerbating other comorbidities, particularly if the patient has preexisting heart or kidney issues.
now imo your electrolytes arent going to change terribly between using mild doses of a diuretic and going into a sauna, but if youre losing water through your skin, its a little better because movement of water/ions out of the skin/periphery is going to be the bottleneck in this system - so if you compare sucking water out of the skin via sweat or out of the blood via the kidney, the ion/water gradient between the blood/periphery is not as prone to being unbalanced of youre taking the fluid out of the bottlenecking compartment
in terms of how good diuretics are as a drug, yeah sure, theyre up there as a potential first line agent in controlling blood pressure. why not? the only bad thing about them imo is the waking up at night issue, but thats hardly an issue with a patient who isnt an idiot and remembers to take them during the day. apart from that theyre relatively benign as drugs
if dakotah had taken frusemide before and had an idea of how it affected her, then sure, why not (apart from waking up every hour to go to the toilet, which really starts to piss you off...pun intended) but on the night before her first comp? when she has a measly 4lbs to lose over 24hrs? in a strength event? nah. bad move, imo.
lol you cheeky bugger, my name isnt spelt t-i-g-e-r-8-8 you knowpintoca said:then again, we know that when people go to your Pharmacy and ask for frusemide, you always say " I don't think is ethically correct for you to have that, so fuck you and the car you came with" and send them their way

pfffffffft thats easy.BrothaBilly said:Ok, let me ask you a question then. You have a 54 y.o. male hx of Congestive heart failure dx, grade 2 systolic murmur, alcohol abuse, longstanding untreated hypertension, atrial fibrillation, ascites, ca++ around his heart on lao chest xray. You auscultate his lungs and hear rawls and he complains of dyspnea on exertion and nocturnal orthopnea graded at three pillows and bilateral edema. P02 of 90%, bp 160/105. He complains of inability to want to eat and you palpate his abdomen and note jugular venous distention, meaning you push on the liver and the neck pulsates.
What is your course of treatment and tests and how would you incorporate frusemide into that treatment, what are your initial suspicions?
Oh, lets throw in there a positive hep C and elevated LFTs as well as recent dental work in the last three weeks preceding his hospitalization for SOB and near-syncope, father has a hx of CAD and Long Qt syndrome, mother positive for breast cancer, no siblings

Big Rick Rock said:BTW- After you weigh in and you gulp down a 16oz bottle of Gatorade without taking a single breath... You will start to sweat your balls off 5-10mins after finishing that bottle. The sweat will be pouring out of your skin for no good reason... That is a sign that you have pushed your body to extreme dehydration. If that doesn't happen, that mean you had a few more pounds of water you could have lost and still been alright.
Good luck homegirl!
