Well, it is not done very often, maybe because of a mis diagnosis. T4 normally does the job. But if the thyroid is malfunctioning to the point of no or very little %4 at all.
In this case, the addition of T3 often helps with many symptoms of hypothyroidism that may not disappear with supplemental T4 only. If T4 is doing the job, then this is of course not necessary. But worth a check.
Always best to armed with adequte questions when you go to a doc. Many docs as you know go stictly "by the book".

Wanted to edit this to note that small dose of T3 like Cytomel may be a good question to ask the doc if it is warranted.
