I've worked in a TV station - those are not 'mistakes'. It's impossible for a porn movie to just "show up" during a super bowl.

This was clearly sabotage, maybe someone wanting to quit or planned this on purpose. Remember, people who work in cable companie's master control, quality control, etc. make like $10/hr or whatever. It's not that hard to press one button and switch router feeds and boom - a new output feed goes out. It can come from anywhere, production truck, effects control, house, anywhere really.

Expect someone at that broadcast center to get read the riot act and fired if they didn't quit already.

I never worked at TV station, but this is the first thing to come to mind for me. How porn ends up on regular by accident seems unlikely.
