What are AAS that are commonly covered by insurance policies?

I know some AAS are rarely covered. I'm wondering if there are certain types of AAS that are covered rutinely by insurance companies in general.
 
That's because so few AAS's are legal for medical use in the USA. Var, deca, and test enanthate come to mind.

My doc has written a 6 mo script of Androgel for me before. With my health care plan at work it ended up costing me $35 a month for 30 packets @ 5grams per packet. Didn't do much at all for me though. Never went back for a refill.
 
Does anyone know how to get a script for var? If I am "recovering" from jaw surgery, and I complain about losing lean mass drastically while still eating a decent amount of food, could I get a script and be covered by insurance?
 
I got Blue Cross/Blue Shield and their formulary only lists test cyp, test enan, test prop, androderm as "preferred" choices meaning it cost me $10 for generic and $15 for name brand.
Anything else costs me like $25. For stuff like halo, Oxandrin, anadrol, winstrol you have to have conditions that those drugs are approved to treat. For example, you won't get Oxandrin unless you are old and they give it to you for wound healing or you have some type of wasting disease like people with full-blown AIDS.
