not sticking to the routine.. if it happens because you were supposed to hit a pr and you fail oh well.. you tried..
also at times if you just can not handle it and stop because of safety ok that can work too..
but again i was talking more about doing more and more reps/sets and more and more weight.. that can lead to trouble more so then taking it easy from time to time.
but it is hard to say exactly without knowing what routine the person is on and other info as well.
