Yes, I almost lost it today. We had a big group (~15 people) in from Russia on a corporate visit. We were leaving the corporate office to see a plant and walked through the gym.

We've got a decent corporate gym and two of the guys wanted to take a picture of themselves (and me). I suddenly realized I was standing in front of a piece of workout equipment with two Russians when "2150 PSI!!!" popped into my head. I swear I almost couldn't stop laughing. They still don't understand why.

They are supposed to send me the picture. If they do, I'll post it.
 
It was interesting to hear them describe a hospital's budget. They get reimbursed per procedure, and we asked what happens if they spend less than they are reimbursed for a surgery. They explained that wasn't the right thing to do. It was twilight-zone worthy.

I had my credit union draft a letter stating in effect, "Mr. PICK3 is pre-approved up to $xx,xxx.xx."

I'd tell the stealership I wood like to give them moar, butt my hands are wut?

If your credit is good enough for them to write you a letter of credit with no LTV stipulations then they should be smart enough to try and flip you to their lenders.

I requested teh limit on the letter I could have gotten moar, I separate teh deal from teh financing ... my loan is my bizness, no credit checks authorized by stealerships (as far as they know I got the $$$ by blowing an old dood)

No, they explained that they get a fixed amount per procedure and are expected to spend exactly that -- No more, no less.

Therefore they have absolutely zero incentive to reduce or even manage cost. They match the quality of care to fit the cost they're given. Just like we will in several years.

you sure cause from your original retelling of the story it sure sounds like they're saying they don't pocket any excess. You may be right in explaining their cost structure but it still sounds like they said no dude, we don't go high and pocket the low. Just sayin. I think the interaction may have been over your head despite being in the room, right next to them infact.

Do you even realize I'm reveling in the fact that you still don't understand the drive to make a profit on each procedure is the same drive they should have to lower their own costs and improve?

That's why I hung post #6 out there in the first place. I knew you'd bite. And in all fairness, I thought Mr. Db might bite on it too, but you swallowed the whole lure from the jump. Mr. Db would have seen it coming and caught himself.

Regardless, I am amused.

not trying to re frame nothing....so far I haven't heard anything from you that say's I missed something. I think you are reframing things and saying you "hung one out there" for someone to bite.

again, we're talking about a socialized medical system where the govt pays them a certain amount per procedure and their compensation is already worked into that initial payment. So you asked them what they do with the money if they happen to go under the amount they were given. Don't test me boy i know the principle of profit making, i just spent all weekend on a feasibility analysis. This isn't the issue though kudo's for trying to deflect. You asked them what they did with leftover money and they said that it's incorrect to not report that.....so if I have the interaction wrong, by all means explain to me that I do. But it sure still sounds like they told you they don't pocket any extra money from procedures and put it back into the system.

i'm still catching the same drift as before....if they're under they give it back but they're making sure they spend what they're given so that the care is as top notch as can be. So in essence their saying they do their best not to short change the individual. It's a socialized system so it's apples to oranges but couldn't you learn something from that squibbles?
