I've used some of the info from Mike Mooney's book (http://medibolics.com/nelson/index.htm)

How do you introduce yourself to the doctors on the list on the above site?

I contacted one of the doctors on the list by setting up an appointment. No problem, no hassle, however, you need to develop a rapport and need, in order to get what you want. Process took about 6 months until we got around to the issue at hand.

Do you mean it took 6 months to muster up courage to ask for a prescription? Was it because you did not have recent lab tests of free/Total-T levels or was it due to doctor not wanting to discuss the issue?

When you finally brought it up, can you share the way that occurred - I mean, just a question from you or a suggestion from the doctor? How was it received?
