How often do you guys crash on the squat rack?

I usually don't have spotters on the squat (there aren't a lot of people in my gym that I trust). Whenever I get stuck I just have to dump it on the safety rack. I miss a squat about once every 6 weeks. Does anyone else do this? I feel stupid when it happens but my creed has always been that if you never miss a squat you're not training hard enough.
 
second time squatting, 155 went up for 1, on 2nd rep got stuck half way and kinda lost it, slipped off a trap and it slammed onto the safety things.
 
I do this twice a week on my ME days. I know you can’t do this in a gym, but I lift alone in my basement. I hung a pair of chains from my power rack. I set them up just below my deepest movement (box squat, GM’s etc…). That way when I lose it... I can just ride it down without risking any injury. Works like a charm.
 
almost never but I usually ask for a spot when going near max
