When taking this when is a good time to take it and do you take them all together or one pill every so often?

Why do some people recomend 10mg first week,20mg 2-3, and 30mg 4th. , Then some say do 40mg first week and go up gradually from 10mg and up 10mg each day. what happens when you just jump right up?
