what do you do when

when you are on your second set and you didnt have enaugh power to do 10 reps so you did 8... on your next set to you pick lighter weight and do more or what?

lets just say you are doing bench press and you want to do 3 sets of 10
 
It depends.

If it is from losing my focus I just try to get refocused and do the set that I was planning on doing. If it was from the weight itself being too heavy, I might drop down and do a set, and depending on how that went I might throw an extra set in with the weight I failed at with no expectations on the number of reps.
 
I'm going to back up Big4Life here about the Focus issue. I think focus and mental toughness is so much more than we realize. For instance, when I do my 5x5 workout my first set is always the toughest and my last always the easiest. Can't explain why, but I'm sure it has something to do with how I mentally see the weights i have to lift.

I don't go for the lighter weight unless I grossly misjudged my first set. I'll keep the heavier weight and do one or two less reps with my goal being to do the same number of reps.
 
after maxing out i always drop straight to like 30 punds lighter to get in as much reps ads i can (after a max its hard to get 5 if i'm lucky)I pyramid up so i am always droppin reps
