serostim lot check for 5mg kit

lot#mok632az exp.08/02 this is a five mg kit has a wafer much smaller than the diamiter of the vial. Has blue flip top with a gold colored ring around the stopper. Can anyone verify this kit. I have never heard to much on the 5mg only the 6mg kits? No holgram but has insert.
