PLEASE help with a new routine

Ok guys been out of the gym for 4 months almost now. Broke my ankle and had to have screws put in. Been of the juice since that time too.

Here is what I want to help get me back started. 2 exercises for back, 2 exercises for triceps, 1 for chest, and 2 for back and legs. I have to start back slowly. Went to the gym yesterday to see what kind of weight I could handle, bench is down to about 300 max, military pressed the 80lb db's 8 times, bent over rows felt comfy with 225 for reps.
