well actually in most cases.. IT IS BOTH.. If estrogen is the main cause there will be considerable fatty tissue around, under and near the gland.. this fatty tissue perpetuates both itself and the gland through the production of aromatase.. aromatase is produced in peripheral fatty tissue.. with some tissues having higher enzyme producing activity than others.
note- there are several other direct or ancillary causes of GYNO..
1. damage to the liver which causes release of growth factors that stimulate growth of the gland
2. estrogen as noted above
3. progestins (not progesterone)-- ER and PR are stimulate breast tissue
4. prolactin (directly stimulates the gland to grow and if enough exposure produce milk)
