How much substance abuse is actually.......

Most of my patients "skipped right over ETOH and went for the good stuff......" (exact quote)

Most of the people I've known with serious drug problems had a serious alcohol problem at some point.
Most of your patients will become drunks when they get clean.

So I just finished my rotation in Emergency Room Psychiatry, and I've been wondering about this. It kinda turns out to be a chicken and the egg question.

1. Are people with undiagnosed and untreated mental illness self
medicating with illicit drugs?

2. How many people had no mental illness until after using
certain illicit drugs?


It's hard to say which came first. Doesn't really matter though, as they treatment will be the same for either.

I definately fell into the #1 category.

It started out as curiosity. But then learned that if i didn't like how i felt(depression, insomnia, boredom,anxiety, etc)i could change how i felt by using certain substances. It is definately an interesting cycle to experience and observe.

I've wondered about this several times. You see the same burnt out "crackheads" or whatever walking the streets talking to themselves and wonder, is it the drugs or the mind?

This is why I couldnt deal with psychiatry. It makes me so sad, especially very young people. And you try and try and try to help them, but as soon as they're discharged, they go out and do the same damned thing, and they're back in three damn days (holy runon sentence batman!).

And then there are those who know is I say the 6 magic words, they get a free bed and meal for a few days. Gahhh, as much as it was really interesting, I'm glad to rotation is over. I liked it, but as a career, I couldn't handle it.

I think that even if it starts out just the drugs, it doesnt take too long before it's the mind..... And it's sad, and I dont want to think that way, but after a while, your brain is not only damaged, but irreversibly rewired.... I think people can take great steps to recover, etc but they are never fully over it.

More often self-medicating than anything else IMO but you're right, can be both.

Yes druggies and wino's ruin the mental health help that could be given to people who really need it. And about the "free bed" for 3 days.... try 3 months...

I would say addictive personalities may attribute to drug abuse but self medicating just sounds like an excuse to me. Either stay weak and continue to use and be shit or get a life. its that simple.
