Eventually it's going to be a lot easier if you tell her. If it's your girlfriend that's one thing - but your wife, eventually who needs the hassle of hiding & explaining away needle marks, acne, etc. etc. etc.

no do't tell her, if you are not up to it. She will throw it in your face the first day you are having a fight with her.
Or she cannot keep her mouth shut, and everybody she knows will know it too. You might as well wear a T-shirt explaining everything to everybody.
