Anabolic steroid use has been well-studied in controlled clinical trials. The best justification for using anabolic steroids would be in someone who is experiencing wasting and has a low testosterone level although that theory has recently been discredited. Opinion is more divided over whether to use it to treat wasting when the testosterone level is normal, but there is no anecdotal evidence that shows wasting can be stopped or reversed with anabolic steroids.
On the other end of the spectrum, there is now extensive use of anabolic steroids for cosmetic reasons (to bulk up). Those who are HIV-positive may say they are using the steroids to "prevent wasting," but that argument doesn't make much sense when they are healthy and have hundreds of T-cells, especially when you consider that many people with HIV infection never experience wasting at all. Anabolics are not without side effects-- they can damage the liver, especially when taken with some of the other medications used to treat HIV infection that also affect the liver. And they may also have neurologic and psychiatric effects, to name a few. I'm not a proponent of anabolic use for verified wasting (though I wish there were more data), but I think people should get their muscles the old-fashioned way.
