Its probably real. Notice his arm in the beginning is normal and hes not trying to hide it to the side or anything, then the camera pans right after the scissors are thrown, its all one take and no break in the action. Plus you can see the scissors under the skin a little. As far as blood there doesnt have to be any immediately after the scissors stick in because the scissors form a temporary plug itself.

It's real but the kid is smart. That's why it looks fake. Average dumb will freak out and/or take the shit out of the arm and cause an hemorrhage. The dude knew he has to calm down, make a laugh of the situation, analize how deep is the cut and proceed either going to hospital or treat himself having all neccesary first aid on hand first.
