Bloodwork: When and how?

Everyone has seen threads or read articles that say you should get regular bloodwork while on. But that is all they say. What types of bloodwork and how often are they needed? We can't just go to the doctor and say, "I'm on a cycle and I need to get some bloodwork done."
 
Total Testosterone
Bioavailable Testosterone (sometimes called “Free and Loosely Bound”)
DHT
Estradiol (specify by the Extraction Method)
LH
FSH
Prolactin
Cortisol
TSH
CBC
Comprehensive Metabolic Panel
Lipid Panel
PSA (if over 40)
IGF-1 (if HGH therapy is desired)


At the one month point, I follow-up with these assays:
Total Testosterone
Bioavailable Testosterone
Estradiol (specify by the Extraction Method)
DHT (if patient is using a transdermal delivery system)
FSH
CBC
Lipid Panel
Comprehensive Metabolic Panel
(if ALT/AST from above are elevated, then get a GGT)

Of course, the list is tailored to each particular patient.

Every six months is good and have them check for stds, lipid levels, kidney levels, and blood circulation. Also have them check your blood pressure levels
