barbell bench pressing question

Just curious, when you lie down on the bench and you're looking up at the bar before you pull it off the rack, where should it be? Like, should the barbell be right above your pecs, your neck, your face?

And then once you pull it off the rack, where should you position it before bringing it down? Right where your abs and chest meet?, your upper pecs, your neck or what.

My bench press has been increasing steadily through the year and as I'm moving beyond 225 pounds I'm starting to really question my form for safety and efficiency.
 
The way I lie down on the bench is that my eyes are staring straight at the bar or they are a little past it but not too far.

I put the bar right where my nipples are and then press up after a pause there. I don't do wide grip, a little bit closer and I keep my elbows tucked in or barley let them flare out.
 
i lay about eyes under bar then lower about around nipple or a inch or so closer in the neck direction. with shoulder width or slightly wider grip
 
A powerlifter will lay with the bar almost behind his head, looking back towards the bar... and lift and bring it well in front of the chest, then descend down towards ur chest, touching at the highest point, then push the bar up and back. Then subsequent reps will go from the up and back position and back to it. Allows for more back recruitment, leg drive and easier racking.
