why is john holmes associated with Deep Throat (the movie)

Maybe it was his first official film and/or helped propell cheesy 70's/elevator music to the forefront?

I thought Harry Reems was associated with "Deep Throat".

Speaking of Reems, here's a quote from an article about him:
