Why do my elbows hurt ?

My right elbow especially hurts, but only when Im warming up. For example I was doing flat bench db presses yesterday. Warm up set for 15 reps about half way through my elbow starts killing me, but when I bump up the weight 15 lbs I dont feel the pain. Can someone explain why this might be happening.
 
Maybe your form changes when you go light. Just make sure you use REALLY strict form, even when going light on the warm-ups.
 
The problem might stem from your shoulders. Elbow pain usually stems from things such as scapula position and humeral rotation.

This pain isnt a normal pain. I think it started a couple years ago when I first started lifting. My form must've been bad because before I knew it I could barely do incline bench or military presses and side laterals were totally out of the question. My shoulders never hurt its only my elbows.
