tried a max 5 on squats today and when i was done my left elbow/bicep feels like it has tendinitis now. my form: grip as close to body as possible, thumbs on same side as fingers, elbows back, wrists straight?
Happened to me once when I was doing lots of squats. I moved my grip out about 2" and loosened my hold just a little which did the trick. You'll have just as much control on the bar as before.

I did this when I started having trouble. It allowed the pain to go away. I have since moved my grip back in, and have not had any more problems.
You may need to make sure your arms and shoulders are warmed up and do some light stretching before you squat. This has kept mine from hurting. These days, I'm finding that I have to spend a lot of time stretching and getting warmed up before I do anything to keep me from hurting myself. Especially in the hips and shoulder area.
