Serious Problem

About a week and half ago I woke up with severe nasuea. Before I threw up I had to urinate. To my horror my urine was as black as it could be. I went to the ER and they did X-ray test, a urine test, and a STD test. I had no pains around my kidneys or in my bladder. They determined I had no urinary infection, no STD, or kidney stones. When all was said in done they simply said I had (spelling) Hemeraturia, "Blood in the urine". Iknew that before I even had gone in. So anyways yesterday I go to the gym and do chest... 4 hrs later when i urinate same thing happened again. When it happened a week and half ago, the night before i had gone to the gym. Is this excersice related? Anyone have anything like this happen to them. Any help would be greatly appreciated. By the way I'm a 24 yr old male 5'10 225 pnds 12%bf.

Dude - If I was in that situation I would make sure to tell the doc EVERYTHING you are taking even vitamins and shit bro dont withhold info in that situation. Wish you the best broski!

Got a visit scheduled with the urologist today, guess im gona tell him my cycle history. Ill let everyone know how it turns out in case it happens to anyone else. Thanks for the suggestions guys.

Well, I went in and they did a bunch of testing at a urologist's office. I may have a kidney tumor. Im going down to boston to see a specialist this monday to find out for sure. So If you guys ever piss black... GO SEE A DOCTOR!!!

Was a urine myoglobin sent? Did they check your creatinine and CPK levels?

Screw the emergency room you need a urologist

Not necessarily. If he has rhabdomyolysis, then he needs an admission, hydration, and strict I/O's to prevent renal failure.

How old are you?

Asymptomatic hematuria in those >50 is a tumor until proven otherwise.

For someone younger, you wonder about other causes. Your "black" urine is probably more tea colored, and could be from rhabdomyolysis from overtraining. By chance did you have extreme muscle soreness?

My dad's at home recovering from kidney cancer op and it really is ok. Aparently we only need a third of our kidney to function base-line. But the other kidney should take over the work of both - increase h20 for sure. Incidently, he had the op done via key-hole surgery so he doens't have such a gapping hole.

Well, if I was peeing dark fluid, I think the ER would have been the right choice, especially accompanied by vomiting.

Not sure if all hospitals do this, but one place I saw had special diets for Kidney patients, a 6 pack of beer / day. Such a deal, beer in the hospital.

Im 24... and it was not tea colored. It was black as black can be. Yes I had soreness the day of my workout which is unusual for me. Whenever I go to the gym the black urine comes back. Each time I go to the gym my strength seems to drop by third. No exageration at all.

Sounds like rhabdomyolysis.

You need your CPK checked (as well as an aldolase).

CPK's >6,000 have a 30% chance of acute renal failure, so be very careful with working out. I've had rhabdo before. It sucks.

my kidneys hurt just from reading this thread.lol ...renal floor patients can be a nightmare sometimes.

Its def rhabdomyolysis. CPK is 10,000 and there were tubular casts in my urine. So right now we are unsure whats causing it. I had a hep a, b, c test done, as well as HIV and mono and enzyme def test although I doubt its that b/c its never happened to me before. Wow this seems pretty scary. I will know more next week. Any experinces with rhabdomyolysis??? all help appreciated.

By the way, with this disease can I be perscribed HGH? I know that HGH will help kidney function and muscle wasting.

Badslinky, the tubular casts in your urine have me concerned. What was your creatinine?

Was an aldolase level sent to look for myositis?

This could all be due to overtraining, but polymyositis should definitely be ruled out. By chance you aren't taking a statin (Lipitor, Zocor, Vytorin, etc.) are you? Maybe I missed this in medical school and residency, but why is your physician checking for hepatitis A/B/C? Were your LFT's also elevated?

I will be leaving next week for an extended mission out of the country and will not be allowed to access EF. If you PM me, I can give you my email for questions.

I have not taken any statins. Other than the casts and cpk being 10,000 I dont know about the otrher test results yet. I do know that he did have me do a aldoase, PO4 test CDK, CPK, LSD and a bunch of other tests. Right now I am simply drinking atleast three quarts of water with three teaspoons of bicarbonate a day to keep my urine basic. Wont know anymore until other tests filter in over next couple of days. I doubt its over training b/c ive trained that hard or even harder in the past but who knows. Thanks for the help.

As far as the heps, I had told him about my steroid and insulin use... he just wanted to err on the safe side and test for everything under the sun that could cause muscle wasting. I had told him that using steroids was something that I did strictly by myself with clean syringes... but you know how docs can be. Dont blame him at all he is a great doc.

I agree with the 3-5 liters of water with the bicarb daily. It's important to keep your urine output high to prevent kidney damage. He should have given you some urine dipsticks to test for heme positivity (myoglobin will cause a positive heme result in a dipstick since it cannot differentiate heme from myoglobin v. hemoglobin).

Well all the tests came back negative which is good/bad. Now we dont have a clue what is causing it.
Excess exercise can cause rhabdo. I treated one of our team members after a rather strenuous mission in Sierra Leone. By the way, their health system is less than desired. We could do more tests on our iSTAT portable lab machines than they could do in the hospital!

Im pretty positive its not the exercise thats doin it, Ive put myself under 10x that amount of stress during college preseason soccer training and never had a problem. Im having a muscle biopsy today. Its the last thing he can think to do. So fqar all the test work has costed 1200 bucks... shit i should open up a lab.

SwatDoc I have done some research on HGH and believe that it has some rebuiling attributes as far as the kidneys are concerned. I have 400IUS of HGH. Wouldnt hurt to take it would it? I mean short of the rhab. being caused by something crazy like cancer I dont see how it would but Im sure you would know. Thanks for your help!
