Question bout thai bag

i bought a banana bag last year, and for some reason, the bolts always seem to back out of the wood and the bag falls down. my question is what is the most optimal way to mount a thai bag to a wood beam? i tried some thread sealant but i'm thinking that when the original holes were drilled they were drilled too big. any help is appreciated!
 
Use a loop of heavy chain around the beam, versus bolting the bag to the beam. My bag has been hanging from a truss beam in my garage for years, and I've had zero issues.
