Interpretation of free test results

35 years old, 5'10" 83kg
Bio available Oestradidal level =104 (ideal male range 40-70) this is copy and paste
Free and active Test=36 (normal 60 to 130 )this is copy and paste
Prescribed Bioidentical T23mg/g P36mg/g
Was feeling very lethargic.

Hey there Oz, welcome...

Whilst we wait for the senior guys to post, from what I see, your free Test is low given the ranges, and your Eest is high given the ranges. Low Test is not always a problem, the problem lies in the balance, or ratio, of Free Test to Free Est. Now, your results, for a male, the balance is the total opposite of what it should be, and your figures are out of range, as well.

So, your Doc should be giving you ancilliaries that will firstly ramp up your Free Test, AND ancilliaries that will bring down your Free Est, thus improving your Test/Est profile...

Can't make out what he has given you, is the first a Test cream, and the second a Progesterone cream? Progesterone is a precursor for Test, but also for EST, and your levels of Est are already high....Given that you got into this position without any outside progesterone help, I would think that adding more progesterone into the mix will only make matters worse...

bio available estradiol = free estradiol (non bound to a protein carrier/SHBG or Albumin) + what is weakly bound to albumin (but still active but less that the free ones) , in other words what is non SHBG (strongly bound and inactive)

free and/or active test = non bound to SHBG or Albumin (the small amount that's non bound to a carrier protein which is about 1-3 % of total test serum)
this amount of test is very active and does most of testosterone's work in the body.

ur bio E2 is very high and need to start an AI right now , Aromasin (Exemestane) , Arimidex (Anastrozol) or Femara (Letrozol).
ur Free T is very low , that's y u r prescribed Bioidentical Test and Progesterone (Progesterone helps to counteract the effects of estrogen on the male body and is the precursor to Testosterone), u have symptoms of low androgens (lethargic) and that's normal it'll take a while but u'll feel better later on.
T23mg/g = 23 mg of test per one gram of cream
P36mg/g = 36 mg of Prog per one gram of cream

how much do u apply per day of each ? (in terms of grams / some creams give u a measurements in cm (centimeter) too each 2 cm = 1 gram )
 
Hi, Xman. Thanks for reply. I apply approx 10mm in diameter by 5mm in height (spoon supplied) once daily. I have no idea about it's weight I will have to ask doc at later stage. Four years ago did a cycle 250mg Sustanon every five days for 6 weeks with awesome results, as a result I have 110 tablets of 20mg Tamoxifen left over. These will be out of date but will still be effective in my opinion. Now the question is what dose? how will it effect me long term?. If I take these and then when doctor orders blood test again and he knows nothing of these tablets there could be certain consequences. i.e lower prescription strength.

The way I understand it (concerning Tamoxifen) is that in reality it should be used in conjuction with another product such as those mentioned by Xman.

The reason for this is the manner in which it works...Tamoxifen does not stop the production of Estrogen, but competes with it at receptor sites only. Therefore your estrogen receptors are full (comparably) of tamoxifen but the Est is still in the body and now has no where to go and the body just produces more Est pushing your levels up even further. Aromasin, Arimidex and Letrozole have the action of preventing the production of Estrogen bringing your total and free levels down...

So taking the tamoxifen could quite possibly skew the results as you fear, but in the wrong direction, not to mention how it is going to make you FEEL (be careful for the Est rebound after discontinuing Tamoxifen as well, due to it having a relatively short half life...)

My Est levels were way high with the last test so I have started by taking 20mg Aromasin eod for two weeks and will then halve the dosage for another ten weeks before discontinuing. Keep in mind Oz that you do need some Est in your system for balance and for that reason Letrozole may be to potent in that it destroys up to 98% of Est in your system. Also keep in mind that suppressing Est for to long brings with it other problems like immune system shut down, joint pain and head aches etc...hence the reason for cycling the Est medication.

Ideally what you want to do is manage your Est levels in such a way that you have a good ratio between free T and free E (not sure what the ratio is, sorry)

Hannes, yes I do understand where you are coming from. After my last post, thought I better do some research on the net because it has been many years since dealing with the above, finding that I am way out of touch. Also to be fair to my doc don't really want to go playing with other substances which could alter results just yet.
 
hannes is right u need an AI (like aromasin) not a serm (nolva) but for the moment don't start it by urself until ur next visit to ur doc and show him ur lab results which E2 looks very high then if he doesn't mention anything about it u ask for an AI (aromataze inhibitor).
