This food-drug interaction was actually discovered by accident. A study that was intending to test the interaction of alcohol and a blood pressure medication, used a mixture of alcohol with grapefruit juice to disguise the taste of alcohol for the study. Ironically, alcohol had no effect on the tested medication, but the grapefruit juice seemed to enhance the absorption of the drug. (4)
The researchers then confirmed the finding by comparing drug levels when taken with either grapefruit juice or water and found five-fold higher blood levels of the drug when it was taken with juice. Orange juice did not show this effect.
Grapefruit juice inhibits a special enzyme (CYP3A4) in the intestines that is responsible for the natural breakdown and absorption of many medications. When the action of this enzyme is blocked, the blood levels of these medications increase, which can lead potentially toxic side effects from the medications.
Research has suggested that flavonoids and/or furanocoumarin present in Grapefruit are the substances that act to block the enzyme in the intestines that normally metabolizes many drugs.

All patients were placed on a low-calorie, low-fat diet which was designed to produce an average weight loss of 1-2 pounds per week. All subjects were asked to consume two servings of Florida grapefruit or grapefruit juice each day. (6)
This study, wasn't particularly valuable in proving whether grapefruit works as a weight-loss aid. There was no control group of people who didn't eat any grapefruit and the weight loss experienced by the participants could perfectly be explained by putting any obese person on a low calorie diet, regardless of whether grapefruit was included or not.
The reason that Grapefruit Diets work, as with many other fad diets is because dieters simply limit their caloric intake to less than 900 calories a day. As soon as you come off the diet, you will gain weight again because firstly these diets do nothing to change long term eating habits and secondly the body interprets a crash diet so low in calories as starvation, hence the body compensates by slowing down metabolism to conserve all available energy. This lowered level of metabolism makes future weight loss much more difficult.
In summary grapefruit has no active properties that burn up fat. It is a healthy, high in fibre and low in calorie food that makes a good addition to any healthy diet as should other fruits and vegetables.

You know some newbie is gunna come along now and ask "So I can inject grapefruit juice along with my steroid juice and get better results?!" And then after that it's the predictable, "Can I mix the grapefruit juice and my test juice in the same syringe, or do I have to keep 'em seperate."

Yeah, I remember it being mentioned way back but haven't really heard much from it lately until it was recently brought back to my attention. Still good for a reminder. I see no reason to disparage it because some vets may already know about it. Whenever I see a post that states something I already know I either expand on it or ignore it.

Dr. David Bailey and Associates at the University of Western Ontario, Canada showed that grapefruit juice acts by blocking (down regulating) the activity (expression) of cytochrome P450 3A4 isoenzyme in the intestinal wall, thereby preventing the presystemic "first-pass" oxidative metabolism (breakdown) for a wide variety of drugs. Grapefruit juice did not significantly block cytochrome P-450 3A4 isozymes in the liver hepatocytes. The net result of blocking this presystemic "first-pass" metabolic pathway, is the potential for a marked increase in the plasma concentration of the drug, often to the point of toxicity.
For example, in the case of a drug called felodipine (Renedil & Plendil) which they were investigating at the time, there was more than a five fold increase in the plasma concentration with grapefruit juice as compared with water.
Other studies with the cholesterol lowering drug, lovastatin (Mevacor), showed that grapefruit juice was capable of increasing the peak serum concentration of this drug up to 19.7-fold (range 5.2-19.7) and the area under the concentration-time curve (AUC) up to 26.3-fold (range 5.7-26.3).
The magnitude of the interaction with grapefruit juice appears to be highly variable among individuals, but the effect is relatively consistent for any given person. In addition, there appears to be considerable interracial genetic polymorphism of the cytochrome P450 isozyme subtypes.
The cytochrome P450 isoenzyme suppression activity of grapefruit juice was found to be accumulative, with an estimated half life at around 12 hours. The suppression effect at the enteric (gut) level, appears to be caused by a degrading (destruction) and not only suppression of the Cytochrome P450 3A4 isozyme. The return of cytochrome P450 3A4 isoenzyme activity appears to require de novo cytochrome P450 isozyme synthesis, further prolonging the effects of grapefruit juice.
Further studies have shown that grapefruit juice will also block other Cytochrome P450 isoenzymes, viz. 1A2 & 3A3. The magnitude of the effects from inhibition of cytochrome P450 1A2 & 3A3 isoenzyme activity by grapefruit juice is relatively minor in comparison to its 3A4 activity, and the clinical significance is relatively unknown.

That is a purist view but as you say it certainly can not hurt
I try also to take some orals on an empty stomach 1st thing in the morning so as to increase absorbtion rate

It's great to see Nelson back and posting on the boards. Somehow he always finds a way to bring old subjects up and make them interesting again or shed some new light on them. Like this thread for example, I completely forgot about the whole grape juice thing, just like I did with drag curls a couple of years back (btw, they gave my biceps a whole new level of growth after being stalled for a while!).
