warmup thoroughly. when i bench i do some light rotator work, maybe some light db presses. stuff to pump a little blood in the upper body. then say i was going to work up to a 405 on the bench i would do something like

315 x 1
365 x 1
405 x1
so basically my jumps are quarter(25), plate(45), quarter, plate, and so on.
if your only working up to say 275 you might make smaller jumps something like
