How often do things like this happen in training?

16 doesn't really qualify for the BS kids classes(unless he has learning problems). He can fall victim to McDojos though, which don't exist a lot in BJJ. Injuries can happen to anyone, but if it's a good instructor, there's not really anything to worry about.

well he has to know when to tap
and he has to be rolling with people that know the difference between training, matches, and a fight to the death
can't find out w/out going tho sadly

I could show you much worse videos from most contact sports - and many non-contact sports (bike riding, gymnastics, diving, etc.) - they are inherently dangerous but chances are you won't be seriously hurt. The other option is to build a bubble around your brother and roll him around everywhere so he's always safe.
