Ok a little history...
After doing quite a bit of hiking and cycling as of late, as well as squats, leg presses, and deads on leg day...my knee became somewhat sore when getting up out of a chair, sitting down, and using stairs.

I went to see my Dr. about this and he feels its a problem of an imbalance in quad muscles pulling the patella to the side a bit which is causing it to rub on a collateral tendon, which seemes to have caused a popping noise in that knee.

He wants me to strengthen the hell out of my Vastus Medialis quad muscle (largest one on the inside of leg closest to knee) targeting it using the quad extension machine one leg at a time keeping my foot pointed outwards at a 45 degree angle. He believes this will pull the patella back over a bit.

He also said no more squatting, but I am consulting with B Fold on a routine including box squats when I am ready so that I can keep that important lift in my routine. I really appreciate his help with this and am looking forward to working with him on this.

I have also started loading MSM, Glucosamine, using capsaicin rub, and taking plenty of Omega 3's as well as a daily soak in the hot tub to attempt to further promote healing.

-Knee is still a little sore, havent done any cardio in about a week.

-did 3 sets of Leg extension...using a mere 45lbs, I brought the weight up with both legs, then held the weight up with one leg holding it up as long as I could with my foot pointed outwards at a 45 degree angle. I did each leg for a total of 3 sets.

-I encountered no pain from doing this, and Im not sure if its placebo or what but it seems to be helping already.

- I have continued to use the calf raise machine with no problems.

-Knee is not 100% but I am optimistic:)

-today I repeated the 3 sets of leg extension, again with no pain or aggitation from it, seems like I get around better on that knee after I do this, but again...it could be all in my head.

-I might back off on this tomorrow in preparation of Box Squats in Friday.

- I also became bold and did my usual 2 mile in/ 2mile out hike today and felt good, no lingering pain and it doesnt seem to have been weakened by this.

Charles Poliquin is a big proponent of VM strength, I believe CoolColJ has links to a lot of that info, you may want to email him.
 
I would look into Active Release Therapy. Correcting a quad strength imbalance is definately a good thing, but you still need to break up the scar tissue and adhesions in the area to restore proper joint mobility. Find the best practitioner you can, preferably one at least level 3 (biomechanics cert). You might be surprised that something in your hips or core area could be impeding your knee's movement.
