gyno question

I asked a similar question a few days back. Regardless, I figured that surgery would remove the glandular tissue that grows in an elevated estrogenic environment. Therefore I assumed that gyno would no longer be an issue, but perhaps only estrogen related fat deposits in the lower pectoral area. However I got only the same sort of answers.

Are surgeons unable to remove all of this specific type of breast tissue and thats why gyno can reoccur? I'm not trying to jack this thread but I feel these are important aspects to consider.
 
how do you tell its gyno?
i mean "puffy" nips? what if youre carrying a little fat? it kinda looks same
also what about the "hard lumps"?
"soreness"?
please can someone help me out as how to "self-diagnose"?
plz forgive if its a stupid question but im not understanding the descriptions

Male breast enlargement (gynecomastia) can recur. Remaining fat cells can put on more fat and shreds of gland can be stimulated to grow. Although any surgery cannot guarantee the return of gynecomastia, I prefer to target the gland first and recurrence has been rare.

Gynecomastia, gyno, puffy nipples is a contour problem typically caused by fat, gland, and sometimes skin. Fat tends to be soft, gland tends to be firm. However, I have found firm fat and soft gland. Fingers of gland typically spread through fingers of fat. You can see what I mean here, roll your cursor over the links to see drawings. Gynecomastia acutally comes in many different forms. Take some time to browse some of the variations I have seen here.
