gyno/insurance paying for surgery

Going through various threads, I see many questions about how to get your insurance company to pay for gyno surgery. Well here is a link to an insurance company manual for when they will pay for gyno reduction surgery:

Looks like with this insurance company, they make you wait for 3 years from the time that you first complain to a doctor! Wow. 3 years of bitch titties and I wouldn't need the surgery, because I would blow my brains out by then.

Yeah, they want bloodwork on adults. That rules out the majority on AAS.
