changing training routines

I have been training for some years now and I'm fully aware that you need to keep changing your workouts. But how often do you guys change your routine? every session? every 3/4 sessions? What I'm unsure whether changing every week is bad or whether a routine should be kept the same for at least 2 weeks or something. I know everyones different, I'm just interested to know!!
 
I rarely change the basic routine that I follow, I just concentrate on adding more weight to the exercises
 
I change every few months or so, but not drastically. However, I've been on the same workout for going on four months now. Normally, I'd have changed things up by now, but I'm still improving (slowly but surely) on my db bench, so I don't want to muck that all up.

Once I level out, hit a wall, I want to try the 5x5 thing.
