Bench form

I hear people talk about bad bench form in terms of injury. What do you all feel is risky form on bench? (ie elbows flairing out, bb style bench etc). Any thoughts?
 
having an arch in your back is safer then not. its more natural because there is a natural curvature to the spine, flattening it out then applying a lot of weight on it is bad, keeping a natural arch is good.
 
Exteral rotation of the shoulders due to not having the forearm directly perpindicular to the bar. It almost looks like a skull crusher and puts a lot of torque on the shoulder. that is why powerlifting bench style is safer for most liufters
 
LS.........when I bench. I try and keep my elbows close to the body as possible but they always flair out when I go heavy. I know its a lat weakness. Thats where the tbar row is gonna come in handy:)
 
It happens to me close-grip, ut on normal bench I use a different approach, not so wide as a Bodybuilder but not so close as a powerlifter, about 45 degree angle down so bar touches at bottom strenum level
