another ER stroy

I was in triage agian the other day going through the motions of all the Bullshit complaints. Then this 24 y/o asain male signs in with short of breath and chest pain. I had just triaged prolly 3-4 bronchitis pts before him so I assumed it was the same. i get him in and start asking questions.

all of his vital signs were normal. he was in pain but no real distress. I was about to send him back out to the lobby until I rubbed his arm inadvertantly. he was cool and clamy.

Something didnt feel right. I kep tthinking this guy is 24 he cannot be a cardiac patient. i was debating what to do in my head so I grabbed a MD and asked them to come in and see him as I get the EKG.

The Md was thinking he is athletic and prolly has something called long QRS syndrome which is what a lot of young people collapse and die of on the track or court.

I hooked him up to the EKG and as it started running across all 12 leads he had ST elevation in several leads. Which is a sign of an acute MI. this 24 year old was having a heart attack.

He got ot cath lab and lived thank God. The cardiologists reported back he sucked a clot out of his Right Coranary artery the size of the end of my pinky.

Holly to young to have a heart attack batman. I almost sent him back to the lobby. I think back and I am not tooting my own horn but there are several nw nurses that do triage where I am work and would have easily sent him back out to the lobby where he may have died.
 
Wow nice job. That is really scary. It's not just old or fat people having heart attacks and strokes anymore.

You too?? Girl sometimes I get crazy anxiety and then I think of other young people that dropped dead and it makes me worry even more. I have had a lot of EKG and stress tests. lol

I've had er visits because because of panic attacks in the past. lol. i always think im the special one having the heart attack young...lol

now i tell myself that I'm not and that it will pass and I wont die....but now I'll be picturing that huge ass clot in McCrotch's story...omg :worried:

It was a good feeling. Sometimes you gotta trust your gut. I have other stories very similar to that too. I am sure many guys in the field have stories as well.

I presume you mean long QT syndrome, not long QRS syndrome.

I've seen quite a few young people with MI's. It's actually more common in those of middle Eastern decent, but can occur in Asians as well as young, fat, inactive Americans too.

Yes the long QT syndrome. I was tired when I wrote this. This guy was of asian decent and a little over weight. This was the youngest AMI I had ever seen.

We had an asian guy come in w cp for a day or so after playing basketball. Went to his doctor the next day and was told to come in to the ER. He came in the next day. He went to cath lab. He came back in 2 weeks later with cp again and had a zipper scar. I was stunned at his age but then again he did wait 3-4 days to come ot the ER
 
are you reffering to the psych pt that died?

i can see how that could happen. it is tragic and you always check a pt on the floor for a pulse.

Sometimes these guys flail on the floor and cause a seen and try and draw attention to themselves to get help faster. Granted they should have checked her when she went down but there is always more to the story.

The youngest I've seen with an acute MI was 18 years old. One of my friends saw a 12 year old with an ST-elevation MI when my friend was finishing up residency.
