It's amazing how much power the mind can have over the body. You can control your breathing to the point that no one notices. It is definetly possible. Plus if he wasn't hydrated, he wouldn't of had to piss and its easy to go 8 hours without shitting.

Doctor must've died. If he wasn't killed, he bled to death.
