5X5 Routine Help

So I like the whole patient lifting theory behind this type of routine and have decided I'd like to set one up in hopes of making gains, I just have a few questions:

1. Should I do a 5X5 rep scheme for each exercise in the workout, or could I do 5X5 for a close grip bench and then do regular sets and reps for the remaining exercises?

2. When picking a weight to use do I need to make sure its a weight that I can do every set or is it ok to go down in weight the last few sets?

3. Whats would be good 5x5 workout I could do once or twice a week to increase my max bench?
 
We have lots of threads on 5x5. Just read up and ease into it all. 5x5 is ideal for the core compound lifts to be done in the 5 sets fashion. Smaller body parts etc can be more like 3x12 AND NOT TO FAIL.

why not to failure? i try to train to failure on my last 2 sets of every excersice and i am maxing out on almost every lift every workout. I've read this isnt good to do but with sufficient rest and nutrition could it be?

Yea do that until your core lifts stall and then either cut down or drop the assistance exercises. 5x5 is great if you use it right...Im gonna have to start it up again
