it sounds like changing the guts in the tank would not be a bad idea at all atleast you will not have to hold the flap open and then you could eliminate that problem

check to see if the flapper is closing and that the tank is filling to the proper level so that there is enough water to flush. once flushing check to see if the flapper is not closing too soon

sounds like there is a blockage usually a plunger will assist with this quite well. if this doesnt help you may need to call a plumber to have your closet rodded shouldnt be too expensive, but if he needs to rod out the drain and pull the toilet then that is more $$$ are you flushing...
