Who honestly plans periodisation into their training schedule?

Who honestly plans periodisation into their training schedule? We all know we should, but to be honest, mine training usually consists of balls to the ball training then a week off every 3 or 4 months followed by 1 or 2 weeks of working back up the weight.

Those that do, what does it look like, and can you tell how has it helped?
 
HST is periodized, however, I will extend the blocks for far longer than advised if I am still gaining in a given block. I see no need to waste them for an arbitrary phase length.

When I was more of a purist, I periodized in Bill Starr's 5x5 and Korte's 3x3.
