You can't blame anyone for that.
The guy squatting had no way of knowing his bone was going to give out. There's no way of preparing your bones for heavy lifts other then to make heavy lifts, which I'm sure he did.
You can't blame the spotters either, this is such a freak event you can't expect anyone to be ready for it. What I understand from reading about this in Powerlifting USA ( I guess they got fooled too ) the spotters were able to stop the bar from pinning his neck to the ground.
