Shoulder injury input!!!!!!

So I really fucked up my shoulder. It constantly hurts and putting on my seat belt kills, mostyl movements where i twist my wrist and others i guess. but since i hurt it at work, the workers comp is paying the bill, it has been two weeks and they still haven't authorized the mri yet. i wake up in the middle of the night in severe pain, can't sleep. when it does not get used for a while it hurts really bad. they put me on soma did no good, an anti inflammatory no good, finally tramadol relieved a little pain. what could be wrong and what is a good pain killer beside tramadol something stronger. i am in severe pain, and what do you think this injury could be?

i can't lift more or less inject it hurts to even type. been fucked for 2 weeks, it seems to be getting worse and i am not using it am i screwed or what
