"The records of this office indicate that you might have an interest in certain property seized by Customs and Border Protection on ******. The property was seized under the provision of Title 19, United States Code, Section 1595a(c), (importation contrary to law) for violation of Title 21, United States Code, Section 952, (unlawful importation of controlled substances) and 21CFR 1312.18.
The property seized is described as follows: **********
Under the provisions of Title 19, United States Code, Section 1618, you may petition for relief from the above liability. The enclosed Notice of Information for Claimants explains your options with regard to the remission of the forfeiture and possible return to you of the seized property. If you decide to petition for relief, you must check the first box on the enclosed Election of Proceeding/Waiver form indicating that you wish this office to consider your petition administratively. Checking the second or third box indicates that you desire Customs to begin appropriate forfeiture proceedings. No matter which box you check, you must also sign and return the form.
All petitions should be filed, in triplicate, at the following address:************. If a petition is not received within 30 days, this office will initiate administrative forfeiture proceedings with no further notification to you.
You may assent to an administrative forfeiture of the property by signing and returning the enclosed form, in which case this office will dispose of the property without futher action."
