Pre cycle blood tests..

Hi all, I'm getting prepared to start my first cycle after months of reading and research. The only thing l need to do before I start is get a baseline levels blood test. How do I go about asking my doc for this? Do I tell him why? Or is there a way to ask for one that u guys commonly use?
