US/UK, not at all likely; other places though....

you don't think all the people ending up with fake gear from TJ/wherever are just buying from guys off the street do you?

There was a story in the paper several months ago about a ring in San Diego busted making counterfeit Rx meds, and smuggling them INTO TJ to sell to the pharmacies to sell right back to the Americans going there for cheaper Rx meds...
